The Foundation for Sarcoidosis Research (FSR) is committed to supporting events that carry out our mission of increasing worldwide awareness for sarcoidosis while raising crucial funds for research. During April, which is Sarcoidosis Awareness Month, our community comes together for the annual awareness campaign. This campaign encourages patients to take an active role in their treatment while also providing resources for anyone looking for answers about this disease. Another campaign goal is to increase public awareness for sarcoidosis. FSR provides patients with resources to spark conversations in their own communities. FSR also encourages people to get involved by hosting third party fundraising events such as a local Team KISS (Kick In to Stop Sarcoidosis) 5k Run/Walk for sarcoidosis awareness. If you have ever thought about being a leader in your hometown sarcoidosis community, then become a walk leader and apply for the 2020 Walk Leader Mini Grant! The FSR 2020 Walk Leader Mini Grant opportunity is intended to support our Team KISS Walk Leaders by helping to remove the barriers from hosting an Awareness Walk/5K during the month of April. FSR will award a limited number of grants with a maximum amount of $250.00 per awardee. To be considered for this award, applicants must follow the FSR’S Walk Leader Mini Grant Awardee Guidelines and include all requested information in the grant application. Funding is limited and will be awarded only to applicants whose application has been approved. Approval for funding is based on the FSR internal application review process. Applicants will be approved or denied within 30 days of their submission. Once the funds for this opportunity are awarded to our Walk Leaders, the opportunity will no longer be available. DEADLINES: FSR will begin accepting applications on December 6th, 2019. FSR will accept applications until all funding has been awarded on or before March 1st, 2020. Funding will not be dispersed until April 1st, 2020, pending approval.
SUBMISSION REQUIREMENTS:
- Must provide Walk Leader contact information
- Walk Name/Event Name
- Number of anticipated attendees
- Itemized event budget
- Fundraising goal amount
- Intended use of funding
- In 500 words or less, provide a description of what the funding will be used for and how you are going to ensure the usage of funds aligns with the organization’s mission?
- Event Description
- In 500 words or less, provide an explanation that includes the following information: 1. Intended use of funding 2. Description of intended results 3. Anticipated attendance for walk
- Marketing Strategy
- In 500 words or less, provide an explanation that includes how you will promote your event within the community and achieve your fundraising goal. Describe how you will reach your fundraising goal if your fundraisers are not meeting their goals?

CONDITIONS
- All applicants must read and understand FSR’S Walk Leader Mini Grant Awardee Guidelines before submitting their application for funding. FSR will sign and send back completed copy.
- Grant recipients will receive a maximum of $250.00.
- The Team KISS Independent Walk Leader Funding Grant is open to the public.
- Each applicant must only apply once.
- Each independently hosted walk will only receive 1 grant.
- A funding grant is for the sole purpose outlined in the request form and cannot be used for any other purpose without FSR’s prior written approval.
- FSR reserves the right to deny any proposal for any reason.
- If the independent walk is for any reason cancelled, then grant awardee must refund FSR the award amount.
REPORTING Grant recipients agree to report on the event outcomes or status of ongoing programs at the timeline outlined below. Reports of 500 words or less should include:
- Short summary of the event
- Expense report along with proof of purchase (see attached)
- Summary of how the funds/materials were used
- Accurate event budget reporting along with proof of purchase
